Succeed. #BeHereLocation: US:WI:MENOMONEE FALLS at our WOODLAND PRIME 400 facility.This job is ON – SITE.FTE: 0.600000Shift: Shift 2Job Summary:Under direct supervision, provides longitudinal support for patients with the main goals of: improving health outcomes including chronic disease control, reducing no-show rates, reducing utilization of the emergency department, and providing patient support after hospital admissions. Provides patient support with self-management by visiting patients in their homes and administering standardized forms to assess care need to support improving health outcomes. Collaborates with the care team to provide each patient with a caring and personalized longitudinal remote and home-based care experience. Assists patients to adopt healthy behaviors, provides culturally appropriate health education and information based on national recommendations, facilitates video visits with the care team, and collects data to help identify health needs.EXPERIENCE DESCRIPTION:A minimum of 2 years of experience working/trained as a Community Health Worker, working with families of diverse ethnic, cultural, and socioeconomic backgrounds is required. A minimum of 6 months of Sterile Processing or Surgical Technologist experience is preferred.EDUCATION DESCRIPTION:High School diploma or equivalent is required.SPECIAL SKILLS DESCRIPTION:Organizational skills necessary to coordinate multiple community resources, schedules, and appointments. Ability to independently prioritize work and the needs of patients. Excellent problem-solving, decision-making, and assessment skills in a team-based setting. Oral and written skills. Ability to work cooperatively in a busy, multi-disciplinary situation required.LICENSURE DESCRIPTION:American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
