They are responsible for planning, management and implementation of projects that support and advance the population health management strategies of Children’s Mercy Integrated Care Solutions (ICS) that include contract compliance, operational strategies, patient centered medical home policies, value-based reimbursement performance and quality and cost measures.At Children’s Mercy, we are committed to ensuring that everyone feels welcomed within our walls. - – Maintains effective channels of communications with providers and networks to promote understanding of policies, procedures and contractual requirements while creating a high level of satisfaction for network providers. Support and lead projects, in partnership with ICS intra-departmental management and staff, that the ICS Network Providers may use in their practice administration. Educates network physicians, mid-level providers, practice managers and their staff on programs, website tools and program/policy enhancements. Explains and interprets contract language as necessary serving as a liaison between the network provider, affiliated health plans, payers and CMICS. Ensures that the external stakeholders (ICS Networks) have a clear understanding of the population health resources, tools and project objectives. Partners with intra-departmental staff to create and monitor works plans and contract objectives. Together, with and ICS internal Population Health programs and subject matter experts, The ICS Population Health and Network Facilitation Representative and other internal departments with which they partner, is ultimately accountable for the effectiveness and success of the ICS Network.
- – Identifies development opportunities and monitoring activities that support population management to include assessment and feedback to practices regarding organizational, clinical and business functions supporting population health management strategies, including utilization of outcome measures. Together with the Population Health project managers, the ICS Practice Facilitation representative determines success of program utilizing the outcome measurements, input from provider and staff, care team members, and continued research on clinical guidelines, and clinical recommendations. Implements changes as necessary to improve program while promoting optimal health system performance
- – Support data analytics, analytical tools/resources, and reports to support quality/cost improvement. This position will review and assure quality and accuracy of data analytics, reports, etc. and works with applicable stakeholders to communicate outcomes and/or modify analyses as applicable in collaboration with internal teams.
Qualifications
- Bachelor’s Degree and 5-7 years experience Quality improvement, population health management and/or health care related field (managed care or provider experience preferred)
- LPN/RN preferred, but not required upon hire Preferred Upon Hire
